If you have a dental emergency and are a patient of record, please call the office immediately.

Initial Consultation
Your initial appointment will include a brief interview including a medical history, and an examination. The appointment usually lasts from 30 to 60 minutes. The doctor will explain your diagnosis and treatment options. Typically, dental treatment will be scheduled for the following visit. You may greatly assist us by providing the following information for your initial visit:

  • The referral slip from your dentist and any X-rays if applicable
  • A list of medications you are presently taking
  • Dental insurance information including any applicable forms

You will be asked to complete a medical history and other pertinent paperwork at your initial consultation. You may print these forms out and fill them out prior to your appointment if you wish. To download initial visit forms as pdf files.

Financial Information
Our goal is to provide the highest quality dental care at the most reasonable fees. Therefore, we ask that payment be made at the time service is rendered. For your convenience, we accept Visa, MasterCard, Discover, American Express, and Care Credit.

Financing options are available through a variety of sources. We offer CareCredit, which is a unique line of credit, utilized for your dental procedures. There are no annual fees and monthly payments can be quite reasonable. The Dental Fee Plan and 3i-Plus Implant Financing are other resources that we have made available to our patients. If you would like to learn more about these programs, please call the office and speak to one of our financial coordinators.

Our office will send you a monthly statement. Many insurance companies will respond to claims within 6 to 8 weeks although it may take longer. Please remember that you are fully responsible for all fees charged by this office regardless of your insurance coverage.

Insurance Information
Our office prides ourselves on working together with our patients to aggressively pursue the maximum reimbursement from dental insurance plans.

Insurance companies have made filing of claims for dental services increasingly complex and the process can be very confusing. Dental coverage differs from medical coverage with regard to yearly deductibles and yearly maximums along with applicable co-pays for dental treatment. We will do our utmost to keep as informed as possible regarding your dental coverage, however, the contract for coverage is between you, your employer and the insurance company. We encourage all patients to keep well informed regarding their insurance and individual policies concerning coverage. Our office will file claims for treatment when applicable and provide insurance companies with all necessary documentation to ensure that your claims receive the highest level of compensation within the guidelines of your plan.

Applicable co-pays are expected for treatment as it is performed. This co-pay is an estimate only as the final coverage from insurance is not certain until payment has actually been received for the claim. Some insurance plans base the amount of benefit on a schedule of fees arbitrarily developed by your employer. The final amount owed directly by you may vary from the plan description or any pre-estimate of benefits. For example, if your plan compensates periodontal services at 80%, it is 80% of the fee contracted by your employer and/or set forth by the insurance company, which may differ from the actual fee charged by our practice. Although we will always be aware of your financial situation with regard to any insurance and coverage, please keep in mind that we treat our patients according to what is in their best interest and not what is dictated by insurance.
